MENTAL HEALTH SUPPORT (available for fee paying clients, as well as NDIS plan-managed and self-managed participants) Oakwood work within a strengths based, person-centred, recovery model of practice. We offer Mental Health Recovery Support, to individuals who experience severe and persistent mental illness and whose ability to manage their daily activities and to live in the community is impacted by their mental health. The ‘recovery approach’ to mental illness is about helping individuals take control of their own life and to live successful and satisfying lives.
Some ways Oakwood can assist participants is by supporting them to :
Better manage daily activities
Skills building
Improve capacity for mental health self-care and self-management
Improve physical health
Develop social and relationship skills
Goal setting
Informal counselling
Reconnect and participate in the community and meaningful activities
Access and referral to other appropriate supports as needed
We work with participants to develop a Personalised Support Plan which outlines their support needs and goals. We will then support and assist participants to develop the skills and resources to achieve the goals. Our focus is on holistic support, empowerment, self-determination and individual's strengths.
This service is an outreach service for individuals 18 years of age and above. Mental Health Recovery Support is a ‘fee for service’ based on an hourly rate $70p/h and available for private paying clients as well as NDIS plan-managed and self-managed participants.
